Automated Patient Reminders Trial Program
Terms and Conditions

By participating in the Automated Patient Reminders  Trial Program (“Trial”) you are consenting to the following terms and conditions (“Terms”):

The Trial is a limited-time free Trial of the Automated Patient Reminders service, made possible through the collaborative effort of TSI Healthcare and PhoneTree.

Prior to initiating phone calls, text messages and emails (“Service(s)”), TSI Healthcare will contact you to review the program, set-up, and details of the Trial. You will not be charged for Services, support, data interface or anything whatsoever during the trial period. You may discontinue the Trial at any time by simply notifying TSI Healthcare in writing of your intent to do so.

Message content and delivery program are determined by the content and availability of patient appointment data and the capabilities of the data interface provided through TSI Healthcare. The Trial utilizes proven scripts, delivery schedules, and methods of delivery. Typical set-up and configuration takes less than two weeks, and integrates smoothly into your daily staff routine.

The trial period (“Trial Period”) shall be for no less than 30-days, during which time Provider shall gather available data to create a return on investment analysis (“ROI Analysis”) to demonstrate the financial impact of utilizing the service(s) in your practice or group.

The Services will automatically be discontinued when the Trial Period ends, with no further obligation. Prior to the end of the Trial Period, TSI Healthcare shall review the results of the ROI Analysis with you, and provide recommendations for continuing the appointment reminder service, and/or other patient notification services.

Should you choose to continue Services, you will have the opportunity to tailor the content, schedule, method and applications to best meet your specific patient communication needs.

Provider reserves the right to limit usage during the free Trial Period.  The Trial program may be discontinued by Provider at any time without cause and without further obligation.
