Important 5010 Transition Information

CMS E-Prescribing Incentive 2012 Payment Adjustment Final Ruling
As part of CMS's Electronic Prescribing (eRx) Incentive Program, individuals or practices that are considered "successful prescribers" in 2011 will receive a 1% incentive payment based on their 2011 Part B-Eligible charges. Successful prescribers are those who report at least 25 unique eRx events between January 1 and December 31, 2011.
Prescribers were required to report e-prescribing activity by applying the G-Code to the claim. In addition, CMS will determine payment adjustments eligibility for 2012 by identifying affirming a prescriber is "successful" meaning that prescriber must report at least 10 unique eRx events between January 1 and June 31, 2011 in order to avoid the 1% payment adjustment in 2012.
CMS originally instituted two "hardship codes" G-Codes to enable prescribers to request exemptions from the 2012 payment adjustment by reporting that one of the following circumstances applies:
G8642 The eligible professional is in a rural area without sufficient high speed internet coverage
G8643 The eligible professional practices in an area without sufficient available pharmacies that are connected for e-prescribing
CMS recently published a final rule which states beginning October 1, 2011 the exemption categories list will be expanded and the deadline by which practices can request an exemption for the 2012 payment adjustment will be extended to November 1, 2011. Prescribers will have the opportunity to attest via a Web-based tool if they meet the following or aforementioned exemption criteria.
Eligible professionals who register to participate in the Medicare or Medicaid EHR Incentive Programs and adopt EHR technology
Inability to electronically prescribe due to local, state, or federal law or regulation (i.e. controlled medication prescribing)
Limited prescribing activity (Prescribe fewer than 10 prescriptions during the reporting period)
Insufficient opportunities to report the electronic measure due to limitations due to the program limitations
Practices should immediately determine if they are subject to the 2012 eRx payment adjustment and if they are, identify if their providers are eligible for any of the CMS approved hardship exemptions. CMS is no longer accepting significant hardship exemption requests for the 2012 eRx payment adjustment via claims however prescribers can report two alternative ways: via the new CMS Web-based tool (for eligible professionals) or via a mailed letter (for group practices that are participating in the eRx group practice reporting option for 2011). To submit exemption through the CMS webpage, visit the Quality Reporting Communication Support Page and to enter the request and supporting rationale. Your request must be submitted by November 1, 2011.
